    Changing a Beneficiary Designation

    You are permitted to designate a person or persons who will receive payment of benefits in the event of your death. You designate a beneficiary (or make changes to your previous designation) by completing a Beneficiary Designation Form. The completed form is to be returned to ING.

    To ensure that death benefits under the Program are paid to the individual(s) of your choice, you should periodically review your beneficiary designation(s) and determine whether any changes are appropriate. This is especially true if you have a life change event (e.g., marriage, birth of a child, divorce or death).

    It is important to note that with respect to the beneficiary(ies) you have designated

  • If you have a change in status, your designation is not automatically updated. For example, if your spouse is your beneficiary, that designation is not void as a result of your divorce. Your ex-spouse would remain your Program beneficiary until you make a change to your designation.
  • The beneficiary you designate under the 457(b) and 401(a) Supplemental Retirement Plans is separate from any designation you may have made under the County’s pension plan or group life coverage.
  • You can choose to designate the same beneficiary under both the 457(b) and 401(a) Supplemental Retirement Plans, or choose different beneficiaries under each plan.

  • If you do not designate a beneficiary, in the event of your death, benefits will be paid to your estate. Please refer to the Program Overview section of this Web site for specifics.

    Changing the Amount You Defer

    To change the amount you contribute (or defer) to the 457(b) Plan, you will need to complete a new Participation Agreement. Click here for a copy of the Participation Agreement.

    Your Participation Agreement will be effective no earlier than the first available pay date of the month following the month in which the Agreement is completed (unless for accumulated pay). You can choose a later effective date.
