The Systematized Benefits Administrators, Inc. (SBA) Online Enrollment website has been designed for easy enrollment into the Mississippi Deferred Compensation Plan & Trust made available through your employer. Before initiating the enrollment process, carefully review the enrollment pages including the investment option summaries, information booklet, current fund fees and expenses which are illustrated in the Investments section of this website.

You should consider the investment objectives, risks, and charges and expenses of the investment options carefully before investing. Prospectuses containing this and other information about the investment company can be obtained by contacting your local representative. Please read the information carefully before investing.

When you are ready to begin the enrollment/change process, simply click here and you will be taken to the on-line form. Complete the form fully online, and then submit it to SBA by clicking on the "submit" button.

To complete the form using paper copy
If you prefer to complete the enrollment process on paper, download the Participation Agreement and complete all the information. Once the enrollment process is complete, please review and confirm your entries prior to submitting your enrollment information to SBA. You may submit your Participation Agreement by either:

(a) Mailing your completed signed form to: SBA, P.O. Box 105, Jackson, MS 39205

(b) Faxing your completed signed form to SBA: (601) 362-4021

Written confirmation of your enrollment along with a complete package containing prospectuses, Plan Document, Question & Answer Booklet, etc., will be mailed to your address.
